The Links between Social Support and Improved Health Outcomes Report *new*
This report highlights the significance of social support as a determinant of health. The report first defines social support, and then discusses current theoretical models, as well as proposed behavioural, psychological, and biological processes linking social support to health outcomes. It then presents current research linking social support to positive health outcomes among senior, homeless and street-involved, and mental health consumer populations.
Planning & Evaluation
- SMART Fund Strategic Plan
This plan describes the Fund vision, mandate, and goals, the guiding principles, key strategies from now until 2014, an implementation plan, outcomes that will be achieved and an evaluation framework.
- SMART Outcome Measurement Guide
Logic models are increasingly being used by funders and non-profits to describe the work they do, the outcomes they plan to achieve and the indicators they will use to demonstrate their achievements. Each SMART fund project reports on a regular basis on the degree to which outcomes are achieved by adopting an ‘outcome measurement’ approach to project planning, management, and evaluation. This handbook, as well as training and one-on-one support provided by SMART fund staff, is designed to assist in fulfilling this requirement. This guide can easily be applied to any community based program and is a free resource for anyone to use.
