Aboriginal Chronic Disease Management
The following projects focusing on Aboriginal-focused Chronic Disease Management and are supported by the SMART fund:
'Operation Sweet Assassin' by Sechelt First Nation
Video Clip
Improves the health and wellness for members of the Sechelt Indian Band living with chronic health issues through support groups that focus on developing realistic self-care management goals.
'Pain Self-Management' by Squamish First Nation
Provides care givers the necessary skills to teach self-administered pain management to members of the Squamish Aboriginal community suffering from chronic disease supporting the use of personal self-care plans.
'Lil'Wat Healthy Lifestyles and Healing' ' by Mount Currie First Nation
Educates community members on chronic disease management and traditional practices which support healthy lifestyle choices.
There is a 4th Program in this category that is funded by Aboriginal Health Initiative Program (AHIP) called ‘Che Che Gah Towlth Helping Each Other’ by the Tla’Amin Community Health Centre
It works to improve the health status of people with chronic disease through increased self management knowledge and information through professional support and exercise.
