Aboriginal Chronic Disease Management

The following projects focusing on Aboriginal Chronic Disease Management are supported by the SMART fund:

'Operation Sweet Assassin' by Sechelt First Nation

Improves the health and wellness for members of the Sechelt living with chronic health issues by concentrating on implementing a community-endorsed Self-Care Management plan, health client support and increased physical activity for clients.

'Pain Self-Management' by Squamish First Nation

Provides care givers the necessary skills to teach self-administered pain management to members of the Squamish Aboriginal community suffering from chronic disease.

'Spiritual Health & Healing' by Mount Currie First Nation

Builds the capacity of Mount Currie people and the Mt. Currie Health Centre team in terms of Spiritual Health, Traditional Healing and Alternative healing methods.

‘Che Che Gah Towlth Helping Each Other’ by the Tla’Amin Community Health Centre

Increases community capacity for managing chronic disease and improves the health status of individuals with Arthritis, Diabetes, Heart and Lung disease by increasing knowledge and access to information on chronic disease prevention, and developing a culturally appropriate program to self manage chronic disease among Tla’Amin community members.
